Step 2: Medical Records
- Cannabis Certification Centers will need to obtain your medical records for the previous 12 months from your doctor. Once we have your medical records, we will schedule your appointment with our medical marijuana doctor. If you do not have access to current medical records please contact us. Some conditions do not require records within the last 12 months. We can also refer you to a doctor locally if you need to be diagnosed.

Step 3: Appointment
- At the medical marijuana doctor certification appointment, one of our experienced medical marijuana doctors at Cannabis Certification Centers will review your prior medical records and consult you on the risks and benefits associated with medical marijuana in Arizona.
- If the medical marijuana doctor qualifies you for an Arizona medical marijuana certification, the marijuana doctor will complete and sign the Arizona Medical Marijuana Certification Form. With the certification form, our staff will assist you with completing and submitting the application for your Arizona medical marijuana card free of charge.
**Insurance companies do not cover medical marijuana certification appointments. Accepted payment methods at CCC: Cash or Credit Card (Visa, MasterCard, Discover)
Requirements for Appointment
- Must be 18 years old or older
- Must have a valid Arizona Driver’s License or Arizona Identification Card (Bring your valid ID to your appointment)
- Must have at least one of the qualifying medical conditions (listed below).
-All conditions require medical records. You must be able to obtain your medical records for the previous 12 months, or be willing to consult with a doctor to bring your records current if needed. We have a referral arrangement for musculoskeletal issues so please contact us for details.
- Initial appointment at Cannabis Certification Centers is $125. Returning patients will receive their annual re-certification (medical marijuana certification renewal) appointment for $100. Your appointment is FREE of charge if you do not get certified by the medical marijuana doctor. Accepted payment methods: Cash, Visa or MasterCard. We charge $25 to assist with the Arizona Department of Health Services (AZDHS) application.Our office staff will take your photo, submit your application, and follow up to make sure its approved.

We Assist You with All Marijuana Card Documentation
- With the Arizona marijuana doctor signed Medical Marijuana Certification Form from Cannabis Certification Centers, we will assist you with applying for an Arizona Medical Marijuana Card (The state charges $150.00 to apply and accepts Visa and MasterCard only). Our fee to assist patients with the AZDHS application is $25. All applications are process withing five business days by AZDHS. The card is mailed upon your approval email, and will arrive with a few days of getting your application processed. When you receive your Card, you will legally be able to purchase and possess up to 2.5 ounces of medical marijuana from an Arizona registered medical marijuana dispensary or medical marijuana designated caregiver in a 14-day period.
